Support at Home includes AT-HM, the Restorative Care Pathway and the End-of-Life Pathway alongside ongoing services. The official services overview links to the separate guidance for each pathway. Treat it as a starting point for the right source, not as one universal set of funding conditions. Restorative Care focuses on regaining function through coordinated care; the End-of-Life Pathway supports eligible people with end-of-life care needs at home. Read the individual guidance for eligibility, approvals and operation.

Give each pathway a clear record
A practical team checklist can capture the approved pathway, purpose, start and end dates, relevant plan and unresolved actions. Keeping these together helps reviewers avoid applying an ongoing-service assumption to a different pathway.
Use chapters 13–15 of the linked manual for supporting detail. Check the current pathway guidance and the participant’s approval before making a service or funding commitment.

How Genoveva could help
Genoveva could help teams keep pathway-specific plans, dates and evidence organised. Separate pathway support is conditional in the proposed product scope: providers should confirm which workflows are implemented and tested for their release before using them.
