A Practical Checklist for Providers&lt;/strong&gt;&lt;/p&gt;&lt;p&gt;Support at Home is not simply a new name for the former Home Care Packages Program.&lt;/p&gt;&lt;p&gt;It changes how providers manage service approvals, participant budgets, contributions, care management, short-term funding and government claims. It also requires care, financial and operational information to remain connected throughout the participant’s journey.&lt;/p&gt;&lt;p&gt;A provider may have individual systems that perform well in isolation but still experience significant problems when information must move between care management, rostering, finance, claiming and compliance teams.&lt;/p&gt;&lt;p&gt;Digital readiness means more than having software. It means your technology, information and processes can reliably support current Support at Home requirements—and adapt when government rules change.&lt;/p&gt;&lt;h3&gt;Is “Support at Home digitally ready” an official certification?&lt;/h3&gt;&lt;p&gt;No single government certification currently allows an organisation or software vendor to describe itself as officially “Support at Home certified.”&lt;/p&gt;&lt;p&gt;Digital readiness is better understood as an organisation’s ability to support its obligations through reliable systems, controlled processes, trained workers and accurate information.&lt;/p&gt;&lt;p&gt;Providers should be cautious about treating a vendor’s “Support at Home ready” claim as evidence of government approval.&lt;/p&gt;&lt;p&gt;A stronger assessment asks:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;Which Support at Home workflows does the system support?&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Which functions are already operational?&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Which functions require manual workarounds?&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;How are government rule changes implemented?&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Can the provider demonstrate how decisions, services and claims were recorded?&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Has the organisation tested the complete workflow from assessment through to payment reconciliation?&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;This checklist is a practical self-assessment tool. It is not an official government accreditation or substitute for legal, compliance or technical advice.&lt;/p&gt;&lt;h2&gt;Support at Home digital-readiness checklist&lt;/h2&gt;&lt;h3&gt;1. Participant identity and program status&lt;/h3&gt;&lt;p&gt;Your system should clearly identify which program and funding arrangements apply to each person.&lt;/p&gt;&lt;p&gt;Check whether your system can record:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;My Aged Care identification details&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Support at Home participant status&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;ongoing funding classification&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;transitioned Home Care Package classification&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;interim or full funding status&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;short-term pathway approvals&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;funding commencement and end dates&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;participant contribution status&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;applicable transitional arrangements&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;registered supporters or authorised representatives&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;A provider should not have to rely on staff memory or free-text notes to determine which funding rules apply to a participant.&lt;/p&gt;&lt;h3&gt;2. Notice of Decision and support-plan information&lt;/h3&gt;&lt;p&gt;A participant’s Notice of Decision and support plan identify the services they are approved to access.&lt;/p&gt;&lt;p&gt;Your system should be able to:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;store the current Notice of Decision and support plan&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record approved service types in structured fields&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;distinguish ongoing services from short-term approvals&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record the participant’s assessed needs and goals&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;prevent or flag services that are outside the approved list&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record when a Support Plan Review has been requested&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;retain previous versions when approvals change&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;If approved services only appear inside a scanned document, staff may have difficulty checking eligibility when planning, scheduling or claiming services.&lt;/p&gt;&lt;h3&gt;3. Current service-list configuration&lt;/h3&gt;&lt;p&gt;The Support at Home service list contains three contribution categories, 14 service types and 49 individual services.&lt;/p&gt;&lt;p&gt;A digitally ready system should:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;maintain the current government service list&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;map services to the correct service type&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;map services to the applicable contribution category&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;retain government service IDs&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;recognise included and excluded activities&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;distinguish the Support at Home service list from the AT-HM list&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record the effective date of every service-list change&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;preserve historical rules for previously delivered services&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;Effective dating is essential. When a rule changes, the system must still apply the correct rule to services delivered before the change.&lt;/p&gt;&lt;h3&gt;4. Personal-care contribution change&lt;/h3&gt;&lt;p&gt;From &lt;strong&gt;1 October 2026&lt;/strong&gt;, approved personal care services will move from the Independence contribution category to the Clinical Supports contribution category for contribution purposes.&lt;/p&gt;&lt;p&gt;Participants will no longer pay a contribution for personal care delivered from that date when they are approved for the service and have available Support at Home funding.&lt;/p&gt;&lt;p&gt;Your system should be able to:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;apply the new contribution treatment from 1 October 2026&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;continue applying the previous contribution treatment to services delivered before that date&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;use the service-delivery date rather than the claim-submission date&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;retain existing personal-care service IDs&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;update individualised budgets&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;display the correct amount on participant statements and invoices&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;support reconciliation where services cross the implementation date&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;This change illustrates why Support at Home systems need configurable, effective-dated rules rather than fixed calculations embedded throughout the software.&lt;/p&gt;&lt;h3&gt;5. Funding classifications and accounts&lt;/h3&gt;&lt;p&gt;Support at Home includes eight ongoing funding classifications, transitioned Home Care Package classifications and separate short-term pathways.&lt;/p&gt;&lt;p&gt;Check whether your system can manage:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;all eight ongoing classifications&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;transitioned HCP classifications&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;quarterly funding allocations&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;interim funding where applicable&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;supplements&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;unspent transitioned HCP funds&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Restorative Care Pathway funding&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;End-of-Life Pathway funding&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;Assistive Technology and Home Modifications funding&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;funding periods, balances and expiry dates&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;These funding sources should not be combined into one unexplained balance. Staff need to understand which account applies and what the available funds can be used for.&lt;/p&gt;&lt;h3&gt;6. Quarterly budgets and carry-over&lt;/h3&gt;&lt;p&gt;Ongoing Support at Home budgets are allocated quarterly.&lt;/p&gt;&lt;p&gt;Participants can generally carry over unspent funds of up to &lt;strong&gt;$1,000 or 10% of their quarterly budget, whichever is greater&lt;/strong&gt;, into the next quarter. Different arrangements apply to eligible unspent funds retained by transitioned HCP participants.&lt;/p&gt;&lt;p&gt;A digitally ready system should:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;establish the correct quarterly budget&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;separate government funding and participant contributions&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;calculate expenditure against delivered services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;forecast the effect of scheduled services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;identify potential overspending&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;identify significant underspending&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;calculate the applicable carry-over amount&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;distinguish ordinary carry-over from transitioned HCP funds&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;show staff which funds are available, restricted or approaching expiry&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;A visible balance is useful, but a forecast balance is more useful. Providers need to understand the likely effect of future services before overspending occurs.&lt;/p&gt;&lt;h3&gt;7. Participant contributions&lt;/h3&gt;&lt;p&gt;Services Australia determines applicable participant contribution rates based on the person’s financial circumstances and contribution arrangements.&lt;/p&gt;&lt;p&gt;The provider’s system should:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;record the contribution rates supplied by Services Australia&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;distinguish Clinical Supports, Independence and Everyday Living&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;apply the rate to the relevant delivered service&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;support “no worse off” transitional arrangements&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record interim contribution rates where applicable&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;update rates without altering historical transactions&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;identify overpayments requiring a refund&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;account for financial-hardship arrangements&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;clearly separate the participant contribution from the government subsidy&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;Providers should avoid manually calculating contribution percentages in disconnected spreadsheets where changes may not flow through to budgets, invoices and claims.&lt;/p&gt;&lt;h3&gt;8. Care plans and individualised budgets&lt;/h3&gt;&lt;p&gt;The care plan and individualised budget should reflect the participant’s approved services, preferences, goals and available funding.&lt;/p&gt;&lt;p&gt;Check whether your system can:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;connect the support plan to the care plan&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;connect approved services to the individualised budget&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record agreed hours or units of service&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record the participant’s preferences and goals&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;obtain and document participant agreement&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;update the budget when services or contribution rates change&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;maintain version history&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;show who approved or changed each record&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;provide the participant with an accessible copy&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;Care and financial information should not tell two different stories.&lt;/p&gt;&lt;h3&gt;9. Care-management workflows&lt;/h3&gt;&lt;p&gt;For ongoing services, 10% of the participant’s quarterly funding is allocated to care management.&lt;/p&gt;&lt;p&gt;Digital readiness should include more than recording that care management occurred. Your system should help care partners complete the work consistently.&lt;/p&gt;&lt;p&gt;Consider whether it can support:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;scheduled participant monitoring&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;care-plan reviews&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;goal monitoring&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;risk and wellbeing follow-up&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;changes in needs or circumstances&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;multidisciplinary coordination&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;participant and supporter communications&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;referrals for Support Plan Reviews&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;tasks arising from incidents, complaints or feedback&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;evidence of actions taken and outcomes&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;Dashboards should help care partners identify what requires attention—not merely display large amounts of information.&lt;/p&gt;&lt;h3&gt;10. Service scheduling and delivery records&lt;/h3&gt;&lt;p&gt;A scheduled service is not the same as a delivered service.&lt;/p&gt;&lt;p&gt;Your systems should be able to distinguish:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;requested services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;approved services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;scheduled services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;completed services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;cancelled services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;participant no-shows&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;incomplete or disputed services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;services ready to be claimed&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;services already submitted or paid&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;The record should include enough information to support the claim and demonstrate what was delivered, when it occurred and who provided it.&lt;/p&gt;&lt;h3&gt;11. Third-party and associated-provider oversight&lt;/h3&gt;&lt;p&gt;A registered provider may engage an associated provider or third party to deliver some services, but the registered provider remains responsible for the services delivered.&lt;/p&gt;&lt;p&gt;Check whether your system can record:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;third-party and associated-provider details&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;service agreements and contract periods&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;required worker or supplier documentation&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;insurance, screening and credential expiry dates&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;agreed prices and applicable overheads&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;services assigned to each supplier&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;service-delivery evidence&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;invoice review and approval&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;incidents, concerns and performance issues&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;the relationship between the supplier invoice, participant statement and government claim&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;A supplier directory alone is not enough if expired documents, pricing and service evidence are managed elsewhere.&lt;/p&gt;&lt;h3&gt;12. AT-HM workflows&lt;/h3&gt;&lt;p&gt;The Assistive Technology and Home Modifications scheme has separate needs-based funding and an approved AT-HM list.&lt;/p&gt;&lt;p&gt;Your system should be able to:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;&lt;p&gt;record AT and HM approvals separately&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;identify the applicable funding tier&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;store prescriptions, assessments and recommendations&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;manage quotes and participant approval&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record supplier details&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;track orders, delivery and installation&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record wraparound services&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;monitor funding and expiry dates&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;retain evidence supporting the purchase or modification&lt;/p&gt;&lt;/li&gt;&lt;li&gt;&lt;p&gt;record completion and participant outcomes&lt;/p&gt;&lt;/li&gt;&lt;/ul&gt;&lt;p&gt;Providers should avoid treating AT-HM as an ordinary service expense. Its approval, evidence and purchasing processes require distinct workflows.&lt;/p&gt;&lt;h3&gt;13.
